Natamycin 5% is the only topical antifungal eye drop — on the WHO Essential Medicines List. It is the treatment of choice for fungal keratitis caused by Fusarium (filamentous fungi — most common in tropical agricultural settings) and Aspergillus. Natamycin 5% is a suspension — not a solution — requiring validated particle size reduction (micronisation) to achieve uniform drug distribution and consistent dose delivery per drop. Natamycin is the only topical antifungal approved specifically for fungal keratitis — a specialty formulation produced by relatively few ophthalmic manufacturers globally. All batches undergo full sterility testing, particulate matter analysis and assay verification.

Natamycin 5% ophthalmic suspension is the only topical antifungal medicine approved by regulatory agencies specifically for the treatment of fungal keratitis (fungal corneal ulcer). No other topical antifungal eye drop has the same level of clinical evidence and regulatory approval for this indication. Its polyene antifungal mechanism — binding to ergosterol in the fungal cell membrane — is distinct from azole antifungals and covers both Fusarium and Aspergillus species, the two most common causes of fungal keratitis.

Fungal keratitis is disproportionately prevalent in agricultural communities in tropical and subtropical regions — Africa, South Asia and Southeast Asia — where ocular injury from plant material, soil and agricultural dust is common. Natamycin 5% ophthalmic suspension is active against Fusarium species (most common in tropical fungal keratitis), Aspergillus species (common in temperate climate fungal keratitis), Cephalosporium, Penicillium and many other filamentous fungi. Standard treatment regimen for fungal keratitis: one drop every 1–2 hours for the first few days, reducing to 6–8 times daily as the infection responds, continued for a minimum of 14–21 days.

Yes. Natamycin 5% is an ophthalmic suspension — not a solution — because Natamycin has very low water solubility. The bottle must be shaken well before each use to ensure uniform distribution of the suspended drug particles. Patients should be counselled on this point. Storage should be between 2–24°C, protected from light.

Natamycin 5% is the first-line topical antifungal for fungal keratitis — particularly effective against Fusarium species. Voriconazole (available as an extemporaneously prepared eye drop in many centres) may be used for Aspergillus or Candida keratitis unresponsive to Natamycin. Most treatment guidelines recommend starting with Natamycin due to its established efficacy, regulatory approval and commercial availability as a prepared ophthalmic formulation.
